The lessee hereby agrees to:
Follow and enforce, any and all rules supplied by LEAPin Little Ones.
Provide adult supervision at ALL times.
Only allow LEAPin Little Ones employees to setup and take down bounce houses.
Return the units and all of its accessories undamaged and in the same condition they were received.
Pay for the cost of any damages, whatsoever, caused to the unit and its accessories, whether as direct or indirect result of using the unit or any other item covered by this contract.
A $35 fee will be charged for each returned check.
I agree to accept the unit(s) listed on this form for use, as is, and to accept full responsibility for the care of all equipment while it is in my possession. I agree to pay full retail value of any equipment rented under this agreement that I do not return. I agree to pay extra rental fee for any equipment returned late. I agree to return all rental equipment by the date and time agreed upon in a clean condition in order to avoid additional charges. I agree to follow all rules stated on this agreement.
LEAPin Little Ones reserves the right to refuse rental service if the location, area, or even the lessee, is deemed questionable and safe.

Acknowledgement of Risk and Release of Liability
DELIVERY:
To addresses specified by the lessee (customer). Lessee grants right to enter said property for the delivery and pickup of equipment at approximately specified times.
ACKNOWLEDGEMENT OF RISK:
I understand and acknowledge that the activity to be engaged in though my rental of an inflatable, interactive amusement device brings with it both known and unanticipated risks to guests, my invitees, and myself. These risks include but are not limited to falling, slipping, crashing and colliding and could result in injury, illness, disease, emotional, distress, death and/or property damage to me or my guests and invitees. All riders will use the inflatable device at his or her own risk.
I voluntarily release, indemnify, hold harmless and discharge LEAPin Little Ones from any and all liability, claims, demands, actions or rights of actions, whether personal to me or to a third party which are related to arise out of or are in any way connected with my rental of the unit including those allegedly attributable to negligent acts or omissions. I agree to reimburse any reasonable attorney’s fees and costs which may be incurred by LEAPin Little Ones in the defense of any such liability claim, demand, action or right of action.
In the event that I file a lawsuit against LEAPin Little Ones I agree that the substantive law of that state shall apply in that action without regard to the conflict of law of that state.
I acknowledge that I have adequate homeowner’s insurance, tenant insurance, or other liability insurance to cover any bodily injury or property damage which might occur to myself, my guests, or my invitees from the use of the unit I am renting.
RULES:
Lessee agrees to supervise both the equipment and its use at all time said equipment is in the possession of the lessee. Accompanying this contract is a set of directions for use and safety rules which I agree to follow and utilize at times during the operation and use of all LEAPin Little Ones equipment.
I acknowledge and certify that I have had sufficient opportunity to read this entire document, that I understand its content and that I execute it freely, intelligently and without duress of any kind and agree to bound by its terms.

RULES
1. Children must be supervised at all times! DO NOT leave children unattended.
2. Ensure that all children using the bounce house understand and follow these rules:
3. Compatible age groups must play on the unit at the same time. Age groups must NOT be mixed.
4. Compatible sized children MUST play on the unit at the same time. Different sized children must not be mixed.
5. The recommended number of children allowed in the standard 15 X 15 bounce house is:
- Ages 3 to 8…….8-10 children
-Ages 9 to 12……..6 children
-Ages 13 to 16………4 to 5 children
6. Bounce houses are designed for children but there is a 1000 pound weight limit Mom and Dad can join in the fun too!
7. The following items are not allowed in the bounce house, and must be removed:
-SILLY STRING!!!
-Food and drink
-Shoes or footwear
-Sharp objects
-Glasses
-Jewelry and watches
-Hats
-Chewing gum
-Party favors
8. NO silly string in, around, or near the bounce house (silly string has a chemical in it that will eat through the vinyl)!
9. No sitting or lying inside the bounce house.
10. No flips or somersaults.
11. Do not bounce or play near the opening of the bounce house.
12. No food or drink is permitted inside or near the bounce house.
13. No running within 10 feet of the bounce house.
14. Keep all pets away from the bounce house.
15. Keep all children away from the blower and power cords.
16. Do not unplug the bounce house while occupants/riders are inside.
17. Do not attempt to move or un-stake the bounce house.
18. Do not used the bounce house under the following conditions, or if they develop:
-High winds (exceeding 15MPH) rain or thunder storms
-Power outage
-If bounce house becomes wet for whatever reason.
-Bounce house is punctured.
19. In the event of foul weather, make sure all children exit the bounce house safely, and then deflate the bounce house by unplugging. Cover the blower to protect it from rain.
20. If the bounce house starts to deflate:
-Make sure all children exit the bounce house quickly and safely
-Check all power connections to make sure nothing has unplugged.
-Check circuit breakers and fuses in house.
-Check blower to make sure it’s not wet.

Waiver Form
Although we strive to make all activities at LEAPin Little Ones as safe as possible, however activities of this nature do come with some element of risk. This waiver ensures that we have contact information of you and/or your child and you are aware of these risks.
A completed Waiver is required to participate in all bounce activities
In consideration of being allowed to participate in any parties or programs with LEAPin Little Ones the undersigned acknowledges, appreciates and agrees:
1. I knowingly and freely assume all such risks, both known and unknown, even if arising from the negligence of the releases of others, and assume full responsibility for all participants listed below; and
2. I willingly agree to comply with the stated and customary terms, rules and conditions for participation. If, however, I observe any significant hazards during my participation, I will bring it to the attention of the nearest official immediately; and
3. The risk of injury from this equipment can be significant, including the potential for paralysis and even death, and while particular rules, equipment and personal discipline reduce the risk, the risk does exist; and
4. I, for myself and on behalf of my heirs, assigns, personal representative and next of kin, hereby hold harmless LEAPin Little Ones their officers, agents, employees, other participants, and sponsoring agencies with respect to any and all injury, disability, death, or loss of damage to personal or property to the fullest extent of the law; and
5. By signing below for my children, and /or spouse. I also agree to the above conditions, should I decide to participate.

Refund Policy
LEAPin Little Ones reserves the right to refuse to complete any event that has the potential to cause damage to its equipment.
Cancellations
Refunds will be given only if cancellation is received 5 days prior to the event or weather conditions (see below) are unacceptable.
Bad Weather
LEAPin Little Ones will not setup equipment in rain, snow, high wind, or muddy conditions. It is strongly recommended that you reserve an alternate indoor location for your event (such as a church or school gym).
In the event of the possibility of bad weather, you will be given the option to cancel 2 hours prior to the event scheduled delivery and receive a refund of your deposit.
If you choose to have us come out and we are unable to set up due to weather conditions (wind, rain, snow, mud) you will forfeit your deposit.
Once setup, the full amount of the rental is non-refundable, even if the event is cut short due to bad weather.
Cold Weather
At temperatures below 40 degrees, the inflatable units can not be set up. If you book an event when there is the possibility of cold weather and you do not have an indoor location, be aware that upon arrival we may not be able to set up the equipment if the air is below 40 degrees.
In the event of the possibility of cold weather, you will be given the option to cancel 2 hours prior to the event scheduled delivery and receive a refund of your deposit.

LEAPin Little Ones POLICY GUIDE
SAFETY POLICY
Safety is our primary concern. With that in mind, LEAPin Little Ones, LLC reserves the right to cancel any reservation that, in the opinion of the LEAPin Little Ones representative, it deems a danger or jeopardizes safety in any way. Upon delivery, we will walk you through safety rules prior to and after set up of the bounce house. The event location must be flat and free of rocks, twigs, sprinkler heads, animal droppings and debris to ensure safety of the jumper and its riders. A 20 foot overhead clearance is needed and a 3-5 foot perimeter around the jumper is also needed. Driveways (or streets for block parties) are fine, just be sure to let us know ahead of time so we bring sand bags instead of stakes to secure the inflatable.
DEPOSIT POLICY
A $50 deposit (Visa, MasterCard, Discover, or American Express), cash or check is required at the time the order is placed. This deposit is refundable as long as the event is cancelled for a valid reason (i.e. weather or family emergency etc.) prior to our drivers arriving at your location for set up (see refund policy).
CANCELLATION/WEATHER POLICY
In the event of bad weather, you will be given the option to cancel 2 hours prior to the event scheduled delivery and receive a refund of your deposit. Equipment will not be set up in the following conditions: rain, mud, snow or high winds. We reserve the right to cancel all bounce house reservations due to inclement weather (rain/snow/wind).
If you choose to have us come and we are unable to set up due to weather conditions (rain, mud, snow or high winds) you will forfeit your deposit. Once any equipment is set up, the full rental price applies.

EQUIPMENT GUIDELINES & USE POLICY
- Customer assumes full responsibility for any damage or loss to equipment.
- Customer is responsible for providing power to operate the blower.
- A grounded three prong plug outlet must be used no farther than 100 ft. away.
-If a generator is the power source, a minimum of 3500 WATT generator (providing 11.6 amps) must be used.
- In case of power failure due to customer negligence LEAPin Little Ones LLC will not make any refunds.
- A responsible adult must be in constant attendance and supervise the proper use of the unit at all times.
- The unit must be fully and securely anchored using all the anchoring points.
- The inflation machinery should be located where it is either not accessible to children and under the direct supervision of a responsible adult.
- All electrical connections should be located in a dry and secure position.
- Fully inflate the unit prior to use. Do not use the unit partially inflated.
- Do not use the unit in rainy conditions or if the wind speed exceeds 20 mph.
- Do not use the unit when it is wet.
- Before entering the unit, all riders should be told that flips, rough housing and climbing the net walls are not permitted, no climbing up slides and sliders must be sitting in a forward facing position, the adult supervisor must strictly enforce these rules.
- If the unit loses power during operation or if there is any other reason to stop the use, the adult supervisor should hold the door open and ask the riders to calmly exit.
- Shoes, jewelry, watches, all pocket contents, glasses, and any other hard or sharp objects should be removed before entering the unit.
- No silly string is permitted to come in contact with the inside or outside of the bouncer, this causes irreparable damage to the bouncer, the customer acknowledges that if the bouncer is damaged by silly string then a $1000.00 fee will automatically be imposed and be due and payable by the customer.
- No pets are allowed in or on the units.
- Do not permit food, gum, drinks or other such substances in or around the unit. A $100.00 cleaning fee will be charged on all units found with food, gum, drinks or other such substances found on them. Please make sure to clean before deflating
- All riders should be in the same group (i.e.: the same related size).
Use common sense in supervising use of the unit. If something does not look right you should stop immediately.

DELIVERY POLICY
Free delivery, set up and take down within 20 miles of our stocking location in Grafton, Ohio. Otherwise the following delivery fee shall be imposed:
21 - 39 miles - $30
over 39 miles: To be determined at time of contract

RENTAL DURATION POLICY
Approximately 6 hours or less will be allocated for your rental period. Rentals will be picked up before 9 pm (or by dark) in order to ready them for the next day.
INSURANCE POLICY
Our goals of providing entertainment and a first class rental experience are followed by our commitment to your SAFETY.
LEAPin Little Ones has made a commitment to safe operation by fully insuring all of our inflatable bouncers in accordance with, and exceeding, the State of Ohio minimum requirements.
In Ohio, insurance is required on all amusement rides, and inflatable bounce houses are considered an amusement ride. An inflatable rental company operating without insurance is operating illegally in Ohio – rent at your own risk!!!!! LEAPin Little Ones would be insured whether Ohio required it or not. We are committed to your safety, as well as the long-term success of our company.
Our insurance covers liability in case of an accident where the party rental company is at fault. It does not typically cover injuries or minor accidents that happen while the inflatable bouncer is under your supervision.
